Instructions for Filling Out Word Forms

1. After downloading the form you must save it you to your hard drive.
2. Fill in the form by clicking on the appropriate checkbox or writing comments where necessary.
3. Print form and fax it, OR email form using appropriate instructions below.

Instructions for E-mailing the Form

1. You've saved the file so now you can open a new e-mail and browse for the file to attach it
2. Please send it to Rosanna Takeda at rosanna.takeda@lausd.net

Instructions for Faxing the Form

1. Print the file.
2. Please fax the form to (213) 241-8030.
